Hate filling out paperwork?  We understand.  We do need all new patients to fill out a history form so that we can learn more about you and your specific eyecare needs and risk factors.  The good news is that after the first year, we can verbally update any changes in your history, so after your first year with us you will never have to fill out the history form again!  If you are a new patient to our office and would like to fill the paperwork out ahead of time at your own pace, you can use the link below to access our history form.  Just don’t forget to bring the history form in with you to your appointment!  Or, even better yet, once the form is completed you could email, fax, or drop it off for us prior to your appointment and we can enter your history form into our electronic health records before your appointment time.
